Sport Physical Information

 

For any students participating in intramural/interscholastic sports at school, Sports Physicals must be done.  If your child has had one done within 365 days prior to try-outs, simply fill out a new Health History Update For Athletic Participation form and give to Mrs. Foti, the school nurse.  If your child has not had a Sport Physical, it is in the best interest of your child that you choose your own MD to perform the physical.  He/she knows your child best.  Please have these forms completely filled out and returned to the school nurse weeks before the try-outs.   For students without a medical home, the School Doctor, Dr. Lara Bruneau, may perform the exam.  Dates for the physicals to be done by Dr. Bruneau will be posted when they become available.  Written consent from you will be necessary. New Sports Physical Forms are below.  It is imperative that all forms be filled out completely, and returned to the school nurse well in advance of try-outs.  A student may not try-out without a completed Sport Physical on file.
